Free Diabetes Education Program

WHO: Open to anyone with a diabetes, pre-diabetes, their family members, or caregivers.

WHAT: The American Diabetes Association identifies this program as meeting the Diabetes
Support Initiative criteria for support programming. Participants will receive the book “Living a
Healthy Life with Chronic Conditions”

Six-week program includes information about:
      Appropriate exercise
      Healthy eating and meal planning
      Monitoring
      Action plans
      How to prepare for sick days
      Complications from diabetes
      Where you can find support and how to be successful
      Building confidence in your ability to manage your health while maintaining an active and
        fulfilling life

Depression Screenings

When: Program runs for four weeks

How: Fill out this Survey.  Your contact information will be given to Mercy Wellness.  They will register you into the program.  Once registered you will get a question once a week about your mood and every 2 weeks you will get the PHQ2.  This will help monitor your mood for a month.

Summary: Many people, and sometimes their families, feel embarrassed or ashamed about having depression. But it isn't a sign of personal weakness. It's not a character flaw. A person who is depressed is not "crazy." Depression is a medical illness. It's caused by changes in the natural chemicals in the brain. Most experts believe that a combination of family history (a person's genes) and stressful life events can cause depression.
