Social Security Number or Legal Name Change

A request to change your social security number or legal name in the Missouri State University System must be made at the appropriate office.

If you are an employee of the University, contact Payroll to report an incorrect social security number. In order to change your legal name on your personnel and payroll records, bring a social security card that reflects your new name to the Payroll Office in Carrington 119.

If you are a student, please refer to the Student Information Change form to change your SSN or legal name.
